Absence from Work Certificates

Your pharmacist at Neighbourhood pharmacy, can provide an Absence from Work Certificate if you need time off work due to:

  • Your own illness or injury (personal leave).
  • Caring for an immediate family or household member who is unwell or injured (carer’s leave).

What to bring with you

  • A valid form of identification (ID).
  • If you need a carer’s certificate, the unwell person should come with you if possible. If this isn’t practical, the pharmacist will use their professional judgement to assess your request.
