Document Certification and Witnessing
Your pharmacist may be able to help you with certifying copies of important documents and witnessing signatures on certain forms. Rules can be different depending on your State or Territory and the type of document, so it’s a good idea to check with your pharmacist first.
What to bring with you
- A valid form of identification (ID).
- If you need a signature witnessed, you need to bring in the printed unsigned form with you.
- If you need documents certified, you need tobring in all your original documents and the copies for each document.
Things to know before your visit
- If a document needs a signature witnessed, it’s important you do not sign it before coming into the pharmacy, as the pharmacist must see you sign it in person as a witness.
- Fees apply.
